Actinic keratoses:
Non-pigmented actinic keratoses on the face often show a 'strawberry' pattern with white circles on a red background. Other features include scale.
Pigmented actinic keratoses can be hard to differentiate from lentigo maligna. Both can show annular granular structures as well as assymetrical follicular openings.
Bowen's disease:
Bowen's disease can also present with a pigmented and non-pigmented form. The classical feature of bowen's disease dermoscopically is the presence of glomerular or coiled vessels.
Pigmented bowen's disease can mimic melanoma. Dermoscopic clues to this diagnosis include surface scale with brown dots and globules (often arranged in lines peripherally).
Bowenoid keratosis share feature of AKs and Bowen's disease often with glomerular vessels.
Squamous Cell Carcinomas and Keratoacanthomas:
Keratoacanthomas often have a central keratin plug. There is usually surrounding thick telangiectasias (usually hairpin or linear irregular) often surrounded by a white halo.
Invasive SCCs often have similar sturcture to KAs with polymorphous vessels (dotted, hairpin, linear irregular..) often with surrounding white halo.
Squamous cell carcinomas (SCCs) and their dysplastic precursors
Pigmented bowen's with scale and brown dots /globules.
A thick vessels with sounding scale and halo in a Well-differentiated SCC.
Pigmented AKs showing scale with fine grey/black dots.
Focal clusters of glomerular vessels with some adherent scale in Bowen's disease.
